E-Check Payment Processing
E-Check payments may be submitted for processing in the following manner:
Click on the E-Check icon below for the type of payment you want to submit (one time payment or multiple/recurring payments) and the form will open in a new window.
Save the form to your computer and then open it with your PDF viewer and print the form.
After entering all of the required information return the completed form to us via email (firstname.lastname@example.org), fax (516-430-5015) or regular mail.
Please ensure that you send us a completed form with all required fields entered. We cannot process incomplete forms.
